Billing Information

The medical billing process can seem complex at times, but Symbios Health strives to make it as easy as possible. We are here to answer all of your insurance filing and billing questions. Please note that unless you are a member of one of our contracted insurance plans or a Medicare recipient, full payment is due at the time of service.

No shows and cancellations less than 24 hours of scheduled appointments will be charged $25.

Accepted Insurance Plans

Symbios accepts most major health plans, as well as Medicare and Medicaid. Please check with your health insurance carrier to verify specific coverage. Other plans accepted include:

  • Aetna
  • BCBS
  • Cigna
  • Medicaid
  • Medicare (and RR Medicare)
  • Tricare
  • United Healthcare

Symbios Direct Care

Symbios Direct Care is a private healthcare membership program that works outside of your insurance, thereby eliminating the expensive costs and erasing the need to file claims. It replaces copays and deductibles with one flat, affordable monthly fee in exchange for specified primary care services such as routine office visits and standard medical testing. Payment Options

Please note that unless you are a member of one of our contracted insurance plans, or a Medicare recipient, full payment is due at the time of service. We accept cash, checks, Visa, MasterCard, Discover, and American Express.

POS/PPO Plans

If the physician is contracted with your plan, the majority of members covered under this type of plan are still required to make some type of payment for service that is rendered to them. This may be in the form of co-payment, deductible, or co-insurance. If your plan has a co-payment, you will be expected to pay your co-payment prior to being seen by the doctor. Co-payments, deductibles and co-insurance are requirements of your insurance plan and we are required under our contract with these plans to collect these amounts from you.

HMO Plans

Most of the members covered under HMO plans owe co-payments. Co-payments will be collected prior to being seen by the doctor. We are required under our contract with these plans to collect these amounts from you.

Balances on Account

All previous balances are to be paid in full within 60 days from first statement sent.

Payment Policy & Guidelines

  1. Please be aware that some services provided may be non-covered services under your insurance policy. It is your responsibility to be aware of the restrictions and guidelines of your policy.
  2. Our fees are standardized based on the length, severity, and complexity of the visit. We will do our best to assist you in identifying all appropriate information in regards to any fees you may have.
  3. Full payment is due when services are rendered for self-pay patients or patients who have no insurance.
  4. If a check is returned to our office because of insufficient funds, the original check amount plus a $25 returned check fee is due. To avoid further late fees or collection action, the new payment and fee must be received within 30 days of the date that the check was originally returned.

Before you arrive for your appointment at Symbios Health.

  1. Please bring your current insurance card with you to every visit. Your physician’s office will file your insurance claim if we are approved professionals for your insurance plan.
  2. In the absence of a current insurance card, you will be asked to pay in full at the office and file with your insurance company yourself.
  3. You are responsible for any amounts not covered by your insurance plan including co-payments, deductibles, coinsurance, and non-covered services. Co-pays and past due balances are due and payable at the time of your appointment.
